Job Description

Essential Functions:

  • Works as a Care Coordinator for individuals in Phases 2 and 3, ensuring their wellbeing and healthcare needs are met.
  • Carries out wellness screenings to gauge the present health condition of the individual. Conducts biometric screenings to measure biological factors.
  • With great proficiency, the individual recognizes and assesses arrhythmias, unfavorable indications, and medication side effects. Effectively communicates and documents any unanticipated incidents to the appropriate staff and/or physicians.
  • Establishes good rapport with the patient and their circle. Instructs and answers inquiries. Expounds on the rationale for the therapy to the patient and their support system.
  • The success of cardiopulmonary health promotion and referral is dependent on accurate identification and effective promotion.
  • Advocates interdisciplinary teamwork to optimize patient care and outcomes.
  • For individuals in Phase 2 and 3, we offer a personalized workout program that is tailored to your specific needs and fitness level.
  • Demonstrates expertise in assessing patients' needs, carrying out effective clinical interventions, assessing outcomes, and making pertinent modifications to care plans.
  • Prioritizes interdisciplinary collaboration as a key component in achieving better patient care and outcomes.
  • You must be able to perform any other tasks that are assigned to you.

Additional Requirements

Required:

Education - Bachelors degree in Exercise Science, Kinesiology, or Exercise and Sports Science

Preferred:

Experience - 1+ years of Cardiology Experience; 1+ years of Healthcare Experience; 1+ years of Rehabilitation Experience

Education - Masters degree in Health Related Field

Certifications - Advanced Cardiac Life Support (ACLS) Certification - American Heart Association (AHA); American College of Sports Medicine (ACSM) - National Commission for Certifying Agencies (NCCA); Certified Cardiac Rehabilitation Professional (CCRP) - American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR).

Physical Requirements:

The role demands medium work, characterized by constantly exerting up to 10 pounds of force and frequently applying up to 25 pounds of force. Occasionally, the job may require using up to 50 pounds of force, and seldomly, a higher weight to relocate objects.
