Position: *Family Nurse Practitioner - MC

Location: Fully remote (never coming onsite)

Duration: 04 months

Duties

  • Electronic Medical Record chart and lab order review, signage and closure
  • Clinical collaboration and oversight of pharmacy colleagues, and technical support via telephone and video-to-video modalities
  • EMR generated lab result report review, patient outreach and results sharing via verbal communication or certified letter publication
  • Partnership with external lab vendors to mitigate missing or overdue lab results
  • Patient-Centered Quality and Safety
  • Evaluate primary care, acute, chronically ill, and transitional care patients, in addition to providing healthcare education and counseling, and disposition planning for our patients ranging in age 18 months and above
  • Provide patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
  • Educate patients on health maintenance and respond to patient care inquiries
  • Document all patient care within an EHR according to MinuteClinic policies and procedures
  • Provide care and coordination of our patients with internal and external colleagues, including the broader patient centered medical home, ensuring the highest standard of care is provided for all patients and at all times
  • Effectively work within a patient care team, including fellow Providers, Collaborative Physicians, para-professionals, Pharmacists and other members of the health care team

Assists in providing clinical services including but not limited to performing COVID-19 point of care testing in a unique setting.

  • Will assist with greeting patients, complete accurate patient documentation via computerized charting, as well as duties such as supply ordering, inventory and weekly patient logs.
  • You will provide clinical and operational support for testing services in a unique COVID-19 patient testing environment, including onsite employer clinics, mobile sites, onsite retail location, and other possible site locations.
  • Will conduct all activities related to screening patients for COVID-19: patient registration in EPIC EMR, insurance verification, point of care test specimen collection/processing, notifying patient of results, educate patient on appropriate next steps based on test results.
  • Additional responsibilities include maintaining infection control, lab processing requirements.
  • You must be able to work within a collaborative team of professionals and at times independently. Will be expected to travel to and staff locations within a manager-defined area as needed by the business.
  • Required PPE will be available as appropriate.

Position Summary

  • The MinuteClinic Nurse Practitioner delivers defined patient care services in a retail clinic environment.
  • You will work in collaboration with a dedicated team of professionals as you independently provide holistic, evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients ranging in age 18 months and above.
  • Care is documented via computerized electronic health record.
  • This position reports directly to the Senior Practice Manager. MinuteClinic is a division of Health. Our clinic practice locations are located inside select Pharmacy and Target Stores.

Education

  • Completion of a: Masters Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role required or in select states, Masters of Science in Physician Assistant Studies (or other health-related Masters Degree in conjunction with Bachelors of Science in Physician Assistant Studies) with current National Board Certification and State of Employment license to practice in the role required.

Job Types: Full-time, Contract

Pay: $78.00 - $81.46 per year
