CAMBA S Brooklyn 3D (Drive Down Diabetes) is a diabetes prevention and home self-management program designed to address the widespread prevalence of diabetes and pre-diabetes in low-income communities of color in Brooklyn. Brooklyn 3D is a multi-faceted program consisting of the following individual, group, and community level components: diabetes self-management education in the home, including personal training sessions; container gardening workshops; and a social marketing campaign.


Position: Nutrition Assistant

What The Nutrition Assistant Does:


  • Provide individual services to assist individuals in understanding the connection of proper nutrition and optimal health.
  • Promote access and maintenance in HIV primary care.
  • Coordinate with other providers, including primary care provider and/or personal trainer about client fitness needs.
  • Assist the program cook in preparing healthy meals based on clients individual nutritional needs, etc.
  • Conduct comprehensive in-home and virtual diabetes knowledge and nutritional assessments, service plans and reassessments with clients.
  • Conduct diabetes self-management education sessions, including blood pressure, glucose, and BMI self-monitoring.
  • Help develop diabetes and nutritional education materials. Monitor clients progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or visits, and document via progress notes.
  • Assist clients in attaining their goals by identifying and locating community resources for clients and by making referrals to appropriate services both within and outside CAMBA.
  • Input client data and client progress information into automated database.


Minimum Education/Experience Required:


  • AA or BA in Nutrition or a related field.
  • Experience working with diabetes; certified Diabetes Care and Education Specialist preferred.


Other Requirements:


  • Bilingual in Spanish or Haitian Creole preferred but not required.
