Pack Health is seeking a Health Advisor Associate to be part of the Health Advising team. As a team member, you will have the opportunity to work with other Health Advisors to solve real problems for our members. You will be involved in all aspects of the member experience. This is a critical position for Pack Health, which is a high-growth company building a new model of delivering health care. This is a role for someone who likes to take ownership, enjoys working in a quickly changing environment, has a great eye for detail, and is energized by making people s lives easier and more productive.   • Coaches members and provides an empathetic, non-judgmental environment to identify effective solutions to healthcare barriers.
  • Works telephonically/electronically with members to manage their chronic condition
  • Interface with member profile and data within Salesforce
  • Develop rapport and relationships with individuals to increase self-efficacy
  • Identify and solve barriers to individual s health management
  • Provide an above and beyond experience assisting members in locating various services, physicians, or resources
  • Monitor data reported directly by members to create a personalized improvement
  • Use KPIs to assess work performance
  • Utilize strengths and interests to work on various projects within the company
  • Being a champion for quality and continuous improvement

Position Requirements:

  • Bachelor s or master s degree in a relevant discipline
  • ​Excellent problem-solving capabilities
  • Excellent teamwork skills
  • Proficiency in some of the following (not all are expected):
  • Customer service
  • One-on-one education/coaching
  • Working knowledge of healthcare systems
  • Prior experience with chronic condition management preferred
  • Experience with patient communication in healthcare setting
  • Hunger to learn and improve through professional development, continuing education opportunities, and national health coach training
  • Ownership mentality
  • Immigration or work visa sponsorship will not be provided
