Beaver Medical Group, a part of Optum is looking for a Full-Time, Nurse Practitioner to join our Senior Wellness Team in the Inland Empire. The clinicians we seek are those who practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient's needs.

SUMMARY: The Nurse Practitioner [NP] or Physician Assistant [PA] is responsible for providing annual wellness visits to patients with Capitated (HMO) insurance.


Position Details:



  • Full-time

  • Schedule: M-F; 8:00 a.m. - 5:00 p.m.

  • Performs annual wellness visits; ability to recognize risk adjusting diagnoses

  • Reviews and orders appropriate testing to meet the 5-star and Quality measures, as outlined by CMS and the Health Plans

  • Examines, assesses, diagnoses and treats patients in a team-based care environment

  • Clinical support staff; rooming patients, vitals, RX refills, and follow up appointments


Required Qualifications:



  • Unrestricted CA License

  • Current CA DEA certificate required prior to start date

  • Board eligible/Board certified

PERFORM DEPARTMENT DUTIES IN A TIMELY AND EFFICIENT MANNER.


ESSENTIAL DUTIES AND RESPONSIBILITIES (Duties include the following; other duties may be assigned.)


Review patient records to determine health status. Recognize risk adjusting diagnoses. Perform physical exams including health history. Performs the Annual Wellness visit, including interpretation of Mini-Cog, depression screening, PFT, monofilament testing and Quantaflo testing. Review and order appropriate testing to meet the 5-star and Quality measures, as outlined by CMS and the Health Plans. Assess patient clinical problems and patient health care needs. Records physical findings and documents a comprehensive assessment of the patient's chronic medical conditions. Assess psychosocial, cognitive and personal assets and limitations of patient; plan care to address these needs. Initiates discussion on advanced care planning and advance directives. Orders, interprets, and evaluates appropriate diagnostic tests to identify and assess the chronic medical conditions. Orders appropriate referrals, immunization and screening tests as medically indicated. Educates patients on their chronic conditions and how to maintain and improve the patient's health. Make referrals within the clinic and to community agencies, services and providers as necessary to meet home care or other health needs. Documents the patient's chronic conditions, including appropriate HCC coding. Communicate with the patients' primary care physician and the coding team. Proficient with the medical group's EMR Perform CPR or ACLS as necessary; have current CPR certification. Consults with supervising physicians per departmental, group, and Medical Board guidelines. Maintains medical records in compliance with Medical Group requirements including but not limited to dictation, written notes, and the use of an EMR. Is compliant with all Medical Group and departmental policies. Other duties as modified to meet the needs of Beaver Medical Group. Ability to travel to other Beaver Medical Group locations to provide Wellness Clinic services. Valid California driver's license required and insurability under the company's insurance carrier standards as applicable to job requirements.
