Terros Health is pleased to share an exciting and rewarding opportunity for a Hospital Discharge Planner working at our Priest Drive Healthcare Clinic in Tempe, AZ .


Hospital Discharge Planners serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site. The Discharge Planner will work with Level I Inpatient Providers to review presenting clinical information, identify resources for successful discharge to the community, and monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider. The Discharge Planner works under the supervision of the Health Center Director or Site Manager.

Terros Health is hiring a Hospital Discharge Planner at our Priest Drive Healthcare Clinic in Tempe, AZ.

Location Priest Drive and Broadway Road, Tempe, AZ


Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

Hospital Discharge Planner Duties include


  • Activities will be performed in both inpatient and outpatient settings.
  • Connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge.
  • Work with BHRs while in the hospital, will work with transitioning BHRs, the hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans as clinically indicated to ensure effective transition to Outpatient or community services, and
  • Reduce the risk of readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and TERROS psychiatric provider.

  • Preferred
    • A bachelor s degree in a behavioral health field.
    • A bachelor s degree in any field, plus one year of work experience in a behavioral health service delivery
  • Must possess a combination of behavioral health education and work experience totaling a minimum of four (4) years
  • Must have valid Arizona driver s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health s driving policy
  • Must have a valid Level 1 Arizona Fingerprint Clearance card or apply for one within 7 working days of assuming role.
  • Must pass a TB Test.
  • Must be in compliance with Terros Health s policy for COVID-19 vaccination
