As a Community Transition Case Manager RN you will have the flexibility to work from home as you take on some tough challenges. You will complete post discharge assessments evaluating members for clinical and social needs timely and effectively, work with other programs and departments as necessary, while collaborating with Primary Care Physicians regarding medication concerns, questions or other member needs.

Schedule: Monday through Friday, no night's weekends, or holidays. The hours are 8:30am to 5:00 pm within your time zone or 4 -10 hour day shifts from 8:30 am to 7:00pm within your time zone at the discretion of the supervisor.

You'll enjoy the flexibility to telecommute from anywhere within the U.S. as you take on some tough challenges.

Primary Responsibilities:
  • Complete post discharge assessments evaluating members for clinical and social needs timely and effectively
  • Follows model of care requirements as outlined in policies, procedures, and job aids
  • Educates members on disease process and medications to include completion Medication Reconciliation - Post Discharge
  • Evaluates members for ongoing needs and completes referrals to longitudinal programs as appropriate
  • Manages, communicates and updates plan of care with members
  • Collaborates with other programs and departments as necessary
  • Collaborates with Primary Care Physicians/Specialists regarding medication concerns, questions or other member needs


This is high volume, customer service environment. You'll need to be efficient, productive and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.

Required Qualifications:
  • Current unrestricted, active RN Compact License
  • 2+ years of case management, managed care, home health, or acute care
  • Intermediate level of computer proficiency with experience in Microsoft Word, with the ability to navigate a Windows environment
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI) with access to install secure high-speed internet via cable in home


Preferred Qualifications:
  • Bachelor's of Science in Nursing
  • Certified Case Manager (CCM)
  • Experience working in a call center environment
  • Experience in discharge planning, utilization review, concurrent review and / or risk management
  • Experience working remotely
  • Bilingual in English and Spanish
  • Reside in Eastern or Central time zone

Colorado, Connecticut or Nevada Residents Only: The salary range for Colorado residents is $54,400 to $97,000 The salary range for Connecticut/Nevada residents is $60,000 to $106,700. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
