Position Overview:

LIFEPlan Care Managers are passionate about providing high-quality services to a caseload of members with intellectual and developmental disabilities through a person-centered planning process. As a Care Manager you will work with members, parents and families, as well as medical and behavioral health, vocational and community specialists to coordinate the creation of an individualized Life Plan that includes all of the supports and services needed members to achieve their dreams. The Care Manager will coordinate, monitor, assess, and work with the person and the support team to integrate supports and services.

Position Responsibilities:

Use strong advocacy and communication skills to ensure members are always at the center of determining their desired outcomes; promote self-advocacy and members civil rights.
Ensure caseload files and health electronic records are compliant, and all documentation and service standards are met, including the completion of activity notes and Life Plans
Utilize technology skills and your ability to learn new software to focus on outreach and coordinating member services
Utilize time management and prioritization skills to achieve multiple member outcomes
Apply empathy, cultural sensitivity, sound judgment and strong attention to detail as you support members and families

Minimum of a Bachelors degree, (preferably in a Health and Human Services field) and at least two years of relevant work experience, OR experience as a Licensed Registered Nurse with two years of experience, OR a Masters degree (preferably in a Health and Human Services field) with one year of relevant experience


Preferably a strong working knowledge of local community resources
Moderate to excellent technological skills, including but not limited to Microsoft Office Suite, Adobe Acrobat and the demonstrated ability to learn, access and navigate web-based systems
Strong verbal and written communication skills and the ability to actively listen to other points of view, work well with diverse populations and treat all people with respect and dignity
The ability to thrive in a tele-commuting culture where you will primarily be working from your home office but will be expected to meet in person, face-to-face with members and parents
Capable of prioritizing your work and structuring your time appropriately
A comfort level with having in person face-to-face meetings as well as teleconference meetings with your supervisor, teams, members and parents, and learning through online training systems and webinars
A valid Drivers License and the ability to meet with members and attend meetings in person in the community and in the office as needed.
Reside in NY state, or a contiguous state
