Responsibilities
The Supervisor- Community Health Work Integration is responsible for implementation and oversight of the integration of community health workers (CHWs) and universal screening for social needs into care delivery pathways, with a focus on screening in ambulatory care and advising integration of screening in acute and emergency settings. The Supervisor- Community Health Work Integration also serves an important role in advancing the integration of CHWs and persons with lived experience in supporting the total health of individuals, families, and communities served by CommonSpirit Health. This includes developing ongoing support for the training and skill development of CHWs, organization of affinity groups and regular conversations among CHWs to share and advance best practices and identify gaps in current training and support programs and achieve program goals and outcomes. Qualifications
Required
Bachelor’s degree in nursing, social work, or other area with/or experience in supporting human service needs of persons and families experiencing vulnerabilities
