The Department of Public Health (DPH) is recruiting for Public Health Nurses II (PHN II) who provide assessments of individuals, families and communities. Duties include case management, coordinating and assisting with access to care; developing treatment plans; providing consultation on medical, dental, and behavioral health assessments; acting as a nurse liaison with public and private agencies; providing assessments and referral for school district students; and conducting disease control measures. Vacancies exists throughout Family Health Services (FHS) and California Children's Services (CCS) sections in DPH.

Family Health Services (FHS) section of DPH specializes in many dynamic public health nursing programs and includes Health Care Program for Children in Foster Care, Rx 4 Kids, Children Screening, Assessment, Referral and Treatment, Black Infant Health, and Maternal Child and Adolescent Health. FHS works in partnership with public and private agencies to provide nursing case management, care coordination and consultation.
The California Children's Services (CCS) section of DPH is a statewide program that arranges, directs, and pays for medical care, equipment, and rehabilitation, when these services are authorized for children and young adults under 21 years of age, who have CCS-eligible medical conditions and whose families are unable to pay for all or part of their care. CCS defines eligibility and selects the most qualified professionals to treat the child's CCS eligible condition.

CONDITIONS OF EMPLOYMENT
 
Travel/Driver License: Travel throughout the county is required. Employees will be required to make provision for transportation. At the time of hire, a valid California Class C driver license and proof of automobile liability insurance must be produced for the individual providing the transportation.
Background Check: Applicants selected for these positions must pass a background investigation and verification of employment history and education.
Continued Education: Additional training/coursework may be required.
Out-of-State Registered Nurses: Public Health Nurse II incumbents who possess a Registered Nurse license issued by a state other than California must obtain and maintain the required CA Registered Nurse license -and- the required Public Health Nurse certificate issued by the California Board of Registered Nursing (CA BRN) prior to hire/employment start date, or contingent job offer will be rescindedContingent job offers may be made to applicants who have provided verification that they have applied for their CA Registered Nurse license -and- Public Health Nurse certificate with the CA BRN, but Public Health Nurse II incumbents may not begin work until both the CA Registered Nurse license -and- Public Health Certificate are obtained. RN must possess a bachelor's degree (or higher) in Nursing to apply for California Public Health Nurse certification. Applicants who do not possess a Bachelor's degree (or higher) in Nursing at the time of application submission will be disqualified.
Applicants must meet both of the following licensure/certification -and- experience requirements:

Licensure/Certification: Applicants must possess and maintain all of the following items:
  • A valid Registered Nurse (RN) license* issued by the California Board of Registered Nursing (CA BRN),
  • A valid Public Health Nurse (PHN) certificate** issued by the CA BRN, -and-
  • A valid Healthcare Provider Basic Life Support (BLS) certificate.
*Out-of-State Licensed Registered Nurses: Applications will be accepted from Registered Nurses licensed in other states, but Public Health Nurse II incumbents may not begin working until the required California RN licensed -and- PHN certificate are obtained. RN must possess a bachelor's degree (or higher) in Nursing to apply for California Public Health Nurse certification. See Conditions of Employment for additional information. 

-AND- 

Experience: Applicants must meet one of the following experience options:
  • Option 1: Three (3) months of full-time equivalent experience as a licensed Public Health Nurse.
-OR-
  • Option 2: Three (3) months of full-time equivalent experience in home health or home visiting as a licensed Registered Nurse or licensed Public Health Nurse.
CCS Section: Experience working with pediatric populations (e.g., Pediatric Intensive Care Unit, Neonatal Intensive Care Unit), which includes inpatient/outpatient intensive care management.

Applicants fluent in American Sign Language, Arabic, Korean, Mandarin, Spanish and/or Vietnamese are strongly encouraged to apply. Additional compensation is associated with positions requiring the use of bilingual skills. Testing of bilingual skills may be required to receive additional compensation.
 
Candidates possessing computer skills in Microsoft applications are preferred.
