Position: Wellness Coordinator (Part-time) HHHN Mission To provide the best health care, and access to that care, for everyone in our communities HHHN Vision To pioneer an innovative, sustainable and community-focused health system through comprehensive primary care and diverse partnerships Proposed Schedule: 24 hours per week. Hudson Headwaters Health Network requires a pre-employment physical that requires proof of vaccinations to include the COVID-19 vaccine, * unless there is a qualified medical exemption Position Summary The Wellness Coordinator is responsible for helping the Food as Medicine Manager implement, coordinate, and assess Network wellness initiatives, and collaborate with community partners and health center care teams for the purpose of promoting and implementing wellness and prevention programs. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily: Essential Duties and Responsibilities: + Provides health coaching, guidance and assistance to patients involved in HHHN wellness programs + Utilize evidence-based guidelines to coordinate and implement programs such as the Chronic Disease Self - Management Program, The Butt Stops Here, and Falls Prevention programs+ Assist in Food as Medicine initiatives that would include patient assessment, Diabetic nutrition counseling and documentation, and assist in nutrition class development and implementation+ Collaborate with community partners (including community organization, businesses, networks, individuals, etc.) and the health center care teams to ensure patient participation with resources, including active communication with primary care providers+ Serves as a guide and provides information/coaching to the patient and/or family for effective healthy lifestyle change, improved self-management skills and participation in available resources+ Participates in measuring clinical outcomes, data procurement and analysis activities + Participates on Population Health work groups as requested Qualifications: + Unrestricted current LPN or RN license to practice nursing in New York State preferred+ Current BLS certification preferred+ 3-5 years experience in a health care related field+ Prior experience with prevention/health promotion and program development and in coaching and/or motivational interviewing in a health-related setting preferred+ Must have a valid driver license and be able to travel throughout the service area to Network health centers when applicable + Strong verbal and written communication skills+ Computer competency, especially as it applies to the handling of electronic medical records+ Well organized with ability to effectively manage multiple cases and projects+ Must be self-directed, detail oriented, motivated and able to work independently as well as collaborate and communicate effectively with colleagues, supervisors, service delivery partners, other health care professionals and co-workers to build and maintain effective, dynamic professional team relationships
