Job Summary: Dietician

The Dietician will work collaboratively with our nurse practitioners, clinical support staff, other healthcare providers and external agencies. Reporting to Enabling Services Senior Manager, the Dietician will be responsible for communicating with patients, guardians, and family members who may have chronic health conditions or health and wellness related questions to close gaps in care and to improve overall health and well-being.

Essential Duties and Responsibilities: Essential Duties include but are not limited to the following. Other duties may be assigned:
  • Assessment of current nutritional knowledge of JPCHC patient populations.
  • Develops custom nutrition curriculum to enhance the knowledge of JPCHC patient populations.
  • Collaboration with external agencies to promote external educational and nutritional programs to benefit JPCHC patient populations.
  • Develops custom nutrition plans for patients based on medical conditions, nutritional needs, feeding constraints, and lifestyle that will optimize nutrition management.
  • Offer counseling and suggest positive alterations in nutrition to address clients' dietary restrictions.
  • Create full and personalized nutrition plans with grocery lists that promote healthy lifestyles based on patient needs or as needed based on chronic disease states.
  • Set clear objectives and provide support to clients to help them in their progress.
  • Develop standardized nutrition assessments and evaluate and develop individualized programs based on results.
  • Work with marketing to promote educational programs on the effects of nutrition on health and fitness through social media.
  • Regularly consults with health care practitioners on nutritional management
  • Maintain consistent records of clients' sessions and nutrition programs.
  • Promote programs within the community and assist with special events as needed.
  • Plans, develops, and conducts seminars, presentations, rounds, programs, and other educational programs as necessary.
  • Maintains professional registration and stays up to date on current thought and research in nutrition and disease as well as disease prevention and health maintenance.
  • Attends pertinent meetings and seminars and reads current literature to provide state of the art care and up to date information.
  • Participates in quality improvement programs and activities in conjunction with CMO and quality team.
  • Assist in the development and implementation of the health center's nutrition care policies, protocols, and standards.
  • Assist Community Services Coordinator with the nutritional values of food to purchase for food pantries.
  • Conduct cooking classes and create recipes.
  • Oversee the Nutrition Nudges program and SWAP within health centers.
Education And/or Experience
  • Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR) or possess certification as Health Educator.
  • Licensed by the Indiana State Dietetics Licensing/Credentialing Board or certified as Health Education Specialist.
  • One (1) year of Registered Dietitian or Health Educator experience preferred.
  • Strong knowledge of nutrition and dietetics, including familiarity with methods of teaching, principles of education, and nutritional requirements of individuals of varying ages and degrees of health.
  • Excellent verbal and written communication skills.
  • Strong organizational, time management and prioritization skills.
  • Strong analytical and critical thinking skills
  • Detail-oriented, able to meet strict time frames
